We know of many Openads publishers serving billions of ads each month (which translates to well above 20M ad imps a day).  At those levels you would have more than one server for ad serving including a dedicated database server.

You're correct that installing Openads gives you an adserver and that the next next step is to find some ads to display.  

It's often useful to start with some ad networks since it's easy to sign up and you can start making money straight away.... but forming relationships directly with advertisers will yield better revenue.
